The syntax for language references dialects is incorrect (and syntactically illegal by BCP47) This patch uses the correct syntax

This pulls in https://github.com/IHTSDO/sct-implementation-demonstrator/pull/2/files from Michael as well as a fix from me which I have squashed to avoid merge conflicts later (assuming upstream merges into one commit)
