Added very draft copy to the top-level "Extensions" section
Changed index.md toc tree to point towards `extensions/index.md` rather than `extensions/dei/index.md`
Added some code to conf.py to fetch extension schemas and add them to the extras folder at build time, so that the docs for extensions build correctly with schema tables
Added some code which checks whether the extras folder exists and creates it if not. This is in response to RTD moaning that it couldn't find the extras folder all of a sudden.

I've reviewed these changes to the DEI Extensions documentation and I'm happy to approve these. They are managed outwith the 360Giving Data Standard governance process.
This PR includes small updates to normative sections of the 360Giving Data Standard, however as this PR is being merged into staging branch for version 1.4, the PATCH process needed to approve these will be managed via the [1.4-staging PR] branch(#369)
