Update index.html

Primarily fix download link and its description to reference the GitHub latest version URL.
Minor tweaks for consistency with README.md.  Lose the RCS ID information.

Update README.md

Add link to https://ronsavage.github.io/SQL and mildly modify the surrounding material.
